OCSRI McAlester: Patient Continuity of Care Transition Plan

Dear OCSRI McAlester patients,

Thank you again for trusting us to be your cancer care provider for the past 24 years. Over the course of our service in McAlester, you have become our family. We are disappointed to be leaving McAlester after serving this beloved community for over two decades.

OCSRI McAlester Location Announcement (As of February 29, 2024)
OCSRI has received notification from McAlester Regional Health Center (MRHC) that our lease would not be renewed as of February 28, 2024. This decision was made without OCSRI’s involvement. OCSRI and MRHC have come to terms on a temporary lease extension through March 31, 2024.

OCSRI Patient Continuity of Care – Transition Plan

Physician visits and treatments scheduled after March 8, 2024, at OCSRI’s McAlester location will be rescheduled to our Tulsa location, or we can refer you to a qualified specialist to continue your care. To ensure continuity of care, OCSRI will be reaching out to scheduled McAlester patients as we get closer to your appointment date.

OCSRI would appreciate the opportunity to continue your cancer care at our Tulsa facility. OCSRI’s Tulsa facility is located at 12697 East 51st Street South, Tulsa, OK 74146, and can be reached by calling (918) 505-3200. The Tulsa location offers the same physicians and services that were previously available to you in McAlester.

ACCESSING MEDICAL RECORDS

Option 1
- To gain permanent access to your medical records, please register for our Navigating Care Portal, if you have not done so already.
Option 2
- If you choose to transition your care to another facility, that facility can request your medical records for you.
Option 3
- Submit Release of Information (ROI) Request Online or In-person
  - Completing the Release of Information (ROI) request form allows us to efficiently process your request and ensure the secure transfer of your medical records.
  - ROI Request Online: You can easily access the ROI request form on our website by following these steps:
    - Medical Records
    - Complete the form and fax it to us at 877-409-6143.
  - ROI Request In-person: You can also visit our office and complete the Release of Information request form in person.
